Birth Certificate

The following specified documents are to be provided at the time of applying for Madhya Pradesh birth certificate:

1. Proof regarding the place of birth (where the child was born) from the hospital.
2. Parent’s identity proof.
3. Marriage certificate of the parent (optional).
